Your Food Allergy Email
Keep caregivers informed about your child’s food allergies.
Dear caregiver’s name,
My child, child’s name, has life-threatening allergies and is allergic to allergens.
Exposure to an allergen can cause a life-threatening allergic reaction, called anaphylaxis. If left untreated with epinephrine, anaphylaxis can be deadly. Since there is currently no cure for these severe allergies, avoiding these allergens is the best approach.
To be prepared, child’s name always carries an epinephrine auto-injector in case of an allergic emergency, which should be administered immediately if you suspect anaphylaxis. Their epinephrine auto-injector contains epinephrine, which can help resolve the symptoms of anaphylaxis within seconds. It can be found in location.
You can familiarize yourself with the signs and symptoms of an allergic emergency at http://bit.ly/4c8yV6F. If you suspect an allergic emergency is happening, or are unsure, administer their epinephrine auto-injector right away and call 911. It has voice instructions that can guide you through an injection and will remind you to call 911 afterwards. After calling 911, please call emergency contact name at emergency contact number to let them/me know what has happened.
I also ask that you watch this training video describing how to inject their device at https://bit.ly/3VLyrfR and practice with the Trainer for additional preparation. You can also download a life-threatening allergy quick reference guide, which contains more information on responding to an allergic emergency, at https://bit.ly/3zK0Xlb.
Thank you in advance for taking the extra steps to help child’s name avoid these allergens. I truly appreciate your support.
Sincerely,
Guardian’s name
